In 2013, CareStar became certified to provide waiver case management services to individuals with Intellectual/Developmental Disabilities diagnoses. Area Five Agency provides case Management services assess individuals who are elderly and/or disabled to determine eligibility for programs that help them remain in their home safely. Our case managers evaluate a person’s ability to handle daily activities with and without in-home assistance. To qualify for program referrals the average monthly cost of in-home care must be lower than the monthly cost of a long-term care facility. The most common services provided include: home healthcare, nursing, respite care for caregivers, housekeeping, home delivered meals, and home modifications such as wheelchair ramps. Area Five contracts with state-certified providers to complete these services. Case Managers maintain relationships with regular home visits and frequent communication with service providers.
